Job Descriptions Duties/Rates

Transport Driver (15 mile limit)
Duties to include pcking up client at the address requested, itis the drivers responsibility to have directions and correct time of pick-up. A confirmation call is required one hour before pick-up to remind client to be ready and cofirm appointment time. Request from client location and time of appointment should be verified by office, with directions accurate. Assist client from wheel chie and into vehicle (please note we do not have lifts or handicapped vehicles), check that clients home is locked and client has keys to home, and paperwork for appointment. Arrival at appointment. Check in the client at ception, and notify them of clients arrival. If the client is scheduled for a return trip, driver may wait with the client (1 hour or less). If return time is unknowm give pager or cell number to reception desk or client may call the office when ready for return. Please not return trip also requires a drop off fee. When complete, take client home making sure they are safe and seeure. call office before lcaving clients home to confirm transfer is complete. Drivers are not allowed to accept money or gifts from client, tips can added to billing and will be awarded to employees on their paycheck.

Housekeeping/Maid Service 
Duties to include heavy ( folded and put away, if requested), carpet spot cleaning, oven and refrigerator cleaning and disinfecting, changing of linens, cleaning bath tubs, basins, sweeping floors and walkways,watering of plants, clening cobwebs and washing walls.

Unskilled Personal Attendant/Companion
Duties to include but not limited to: assisting in all daily needs of ambulatory patients. Assist with bathing, dressing,hair, make-up, and shaving. Assure client safety when ambulating, or walking with a cane or walker. Exercise client or take for walk. Prepare meals and chart nutritional intake, assist client with eating if necessary. Supervise clients medication, grocery shopping, light housekeeping (vacuum, dust, dishes, laundry, water plants, and care for pcts). Keep a log for changes and informations regarding clients health and conditions, wwhere it can be family or visiting nurse.

Skilled Attendant/HHA/CNA
Duties to include but not limited to: Ability and willingss to care for all Non-ambulating lients.Hoyer Lift,Assisted/Non-Assisted transfers. Knoweledge of catheter care, and incontinence. Bowel and Bladder training, intake outtake of fluid logs. Ability to log blood pressure, temp, and pulse. Ability and knowledge of Oxygen, Brcathing Treatments. Have working knowledge of diabetes and glucose testing. Ability to care for Hospice clients. Ability to care for bed bo nd clients (sponge bathing, and feeding, positioning, with knowledge of Range of Motion, and skil integrity). Skilled attendants are expected to complete duties listed above as well for Non-Skilled presonal as well.

Wake Up/Tuck In/Shower Visits
Provide AM or PM care as requested by clients, includingn all personal care and and liner changes, meal preparation, if necessary. Vital signs, Charting and reporting any changes in conditions to supervisor or family member. These services available in certain areas only.
